DOCUMENT CHECKLIST

After indicating interest, we will evaluate your application. If found eligible, you will be contacted by one of our Agent Supervisors and required to provide the following documents:

• Duly completed Application Form 
• Duly completed Assessment Form 
• Copy of Corporate Affairs Commission business registration papers and Tax Identification Number (for registered businesses only)
• Passport photograph
• Copy of valid Identity Card (National ID, Voter’s card, International Passport or Driver’s license)
• Written proposal by the Bank– acknowledged by you with each page signed.
• Duly completed Agent Agreement – acknowledged by you with each page signed
• Guarantor’s Form
• Three months bank account statement
• Copy of shop Rental Agreement/Lease Agreement and Utility Bill
• Photographs showing the business location of the prospective agent (Inside view, outside view, sales point view, vicinity/environment view)
• Sketch showing the business location with nearest landmark
